While in the SFF program, CMS expects facilities to take meaningful actions to address the underlying and systemic issues leading to poor quality.
LWCI can provide facility specific, expert consulting services to address the most critical focus areas each month, while holding the Interdisciplinary Team accountable for improvements.
PROGRAM OPTIONS
Onsite VS. Remote
While the best practice might be to spend time onsite, this option is always going to be more costly due to travel time and expenses. LWCI is willing to perform work remotely. The client might want to have an INITIAL ONSITE VISIT, followed by virtual communication and uploading documents to a secure portal. The INITIAL ONSITE VISIT can be done totally through virtual communication and use of the secure portal.
Establishing a Baseline
LWCI recommends a baseline Initial Assessment of 8 hours to review critical areas that need to be FIXED, followed by a 1 hour plan to the client team. This typically costs around $2,250.
Ongoing Support
Each month, LWCI recommends a package to include 3 hours per week of consulting as a baseline, with outcomes, recommendations, and accountability assignments to be reviewed weekly with key team members on a virtual platform for an additional one hour/week. A typical package of these services costs around $4,000 per month.
